Why are you selecting "OTHER"? You should instead be selecting "Microsoft EXCHANGE" for the account type.
There should be no incomming and outgoing server configured separately.

I have used the "other" option on the new mail setup and entered:
name
e-mail address
AD password (Exchange)
incoming as mail1.domain.net (our mx record is mail1 to resolve to our IP)
user server\user
PW
outgoing: smtp.domain.net
IMAP, SSL, nothing connects. Apple and AT&T have no info after nearly 6 hours with them. OWA, etc. all work fine through Safari as well.
