Speaking from experience, if you plan on tethering, use it sparingly.
Otherwise you might be in for a headache.
While AT&T might say you have "unlimited" data, it's ultimately up to their discretion. Don't think AT&T is beyond sending you an outrageous bill for usage that is out of the ordinary. Around three months ago, one of my friends had his tethered to this laptop, and used it daily for the better part of a few weeks. When it was time to pay, the bill was near $1000.

I'm assuming you're using a jailbroken iPhone.
Technically they can't find out, as the traffic will be exactly the same. But since they're hungry for money and stuff, it is possible that if you're bandwidth usage is unusually high for an iPhone user, they check your traffic more thoroughly, and by detecting usages that's not possible by an iPhone (Like online games, torrent and stuff, flash file downloading) they decide that you're tethering. Anyways, they won't blame you for tethering as that can't be proven, but I've header telcos to charge someone for using too much bandwidth or limiting they're monthly bandwidth.
