Create Survey Form in Filemaker Pro

Any code sample on how to create a survey form using filemaker?
Like most survey form, you have radio buttons for users to select either "Like", "Don't Know" or "Don't like".
End of day, i need to do a survery result page where i need to count for question 1, how many ppl select "Like" option, "Don't Know" option and "Don't Like" option.. and etc
