I would suggest that you run the Act diag repairs as above but if you didn't want to submit your database for analysis you could attempt to create a copy of the database to try and rule out any issues.
File -> Save Copy As -> Choose the option to Save an Empty copy.
This will have none of your data in it but will allow you to test and see if you have the errors.
If the errors still persist with this copy it would suggest that the database back-end has some issues and you can either choose to send of the database for repair or re-build the database by creating a new database and adding your fields.
If there are no errors export the data from your existing database to the new copy, File -> Export and then choose to export to an Act database and follow through the wizard.
Again check to see if you have the errors, if so it would suggest an issue with the data rather than the database.
