Script to prompt for information for contract

I have several contracts which are used by many people for construction and design services. The boiler plate has scattered throughout it, many paragraphs that require project specific information. I would like to automate the process and prompt the user for specific information. At the highest level, there are the senior administrators that have imput, project managers that have imput and contract administrators that have input. So, each user should have have a succient set of prompts for information.
My question is how would you do this software wise? We use Microsoft Access and perhaps can use mySQL for a dB if that is part of the solution.
