Disappearing Records

Disappearing records:
I am using VFP 9 with FPW2.6 dbf files (they are NOT in DBC and are not Visual Tables - they are OLD STYLE standalone DBF).
I have been noticing that every once in a while a record will completly disappear. There is no evidence if was deleted - in fact the delete command is not used anywhere in the executable. I do use multilocks and table buffering on these DBF using the following code (see snippet)

this code works - evidenced by the fact that after the process had been complete printed copy of the purchase order is provided and the print routine requires information from the pmast record.

However, later (from a few weeks to a month or so when the vendor's invoice arrives the user can no longer print the invoice becasue there is no evidence of a master record.
When I search for it - both with deleted() = ON and OFF, there is no evidence the record ever existed.
There is no pack in the program anywhere.
The DBF does not appear corrupted.
What things can cause a record to completely disappear. I am already looking at the code to see if there is a way for it to be overwritten - but save that - is there any outside force that I could also look into?
