"Records should not depend on anything other than a table's primary key." Untrue. Irrelevant even.
"Boyce Codd Normal Form (BCNF), and fifth normal form do exist, but are rarely considered in practical design." That's pretty poor advice. Ten years ago people thought that way. There's no excuse for it today.
Just to clarify the forms that exist are 1,2,3,BCNF,4NF and 5NF (done in that order)
1Nf to 3Nf look at dependancies of attributes on the Primary Key.(plenty of examples on the net)
BCNF occurs if every determinant is a candidate key. If not split the tables and join with FK.
4NF looks at independant multivalued dependencies (if they are independant then you can split the table, if they are related then keep the table)
5NF looks at splitting the table further. if the data is the same after you have split the table and rejoined (i.e. you do not end up with any spurious tules(extra rows)) then the original relation was not in 5NF(so you split the table to make it in 5NF)
