Help with PHP form to MySQL Database

Alright, I've got my form up and working...but only partly. The form is located here: www.jayne-sells.com/pages/form.html. When I hit submit, it posts to my database but only some of the information shows up correctly and I'm not sure where to start on fixing the problem...

For example, none of the 'subjects' checkboxes show up in the database at all. The 'publishers' group will only show one of the checkboxes even if I check all of them. And, the date is showing up as 0000-00-00 00:00:00

I know this is going to be an easy fix for some of the veterans so thank you for the help in advance.Start Free Trial
