mysql insert into datagrid problem

After the .aspx html form passes three fields to be inserted, sometimes the insert method works and other times it does not. I then have to click on the 'add record' button a second or third time. I've tried using the 'trace = true' to follow the logic but cannot see the problem. I 'm new to asp.net C# and maybe it's a lack of understanding the implementation method.
