Context schema

I have some queries on ctxsys schema . i have read the metalink Note:98479.1 . I need some additional information in which is explained in more details.

To my understanding in context schema i can create index on the lob columns . However if i have a column in table in some other schema then can i create a index on that in ctxsys schema. Kindly explain in more detail the procedure to create the index as when i m following metalink doc it gives me  lot of errors.
