Did you try to create the database once already and it failed? If so, do a shutdown abort first, then try creating the database again.

Apart from above comment, You can check how many database you have in window environment by opening control Pannel/Administrative Tools/Services then look for OracleService<database_name

One possibility is that you are trying to run a script from SQL*Plus in a session that has already done an instance startup that included mounting (or opening) an existing Oracle database, but the script you are running includes an "alter database mount" or "alter database" open command.
Another possibililty is not having the value for the environment variable: ORACLE_SID set correctly.
Which O/S do you have? Does the server already have one (or more) Oracle database instances running on it, or are you trying to create the first one?
