I'm not familiar with Oracle forms, but there is a bug report in Oracle Metalink (search for Note 399635.1) regarding something similar to your issue:
Subject: Internet Explorer Crashes When Forms Is Launched
In particular, there is a problem related to 3rd party browser extensions. "At present there are some extensions that may cause conflicts with Jinitiator. These are:
Windows Live Toolbar, Windows Live Toolbar Helper, Windows Live Messenger, McAfee Site Advisor"
If you have any of those installed, try to disable them.
From the same link:
IMPORTANT: The Microsoft JVM reached end of life at the end of June 2009. As such customers should consider moving any non Oracle Applications off MS JVM to Sun JRE. Making such changes would also avoid conflicts.

Its simple. As seen in the error, The module is JVM.DLL which relates to Java Virtual Machine. d/dt_verif y.jsp?plug in=true& la test=true& users_jre= 1.6.0_14
http://java.com/en/downloa
^ go there and download the latest version. Post download/install, click on "Test your JVM" to make sure it works well.
Also, for precaution in the IE browser, go to tools > Internet options > Under General - delete cookies and files., Under Programs - Restore Web Settings, Under Advanced - Restore Defaults and uncheck "enable third party browser extentions (which will require a restart)"
Let us know if this works or not. Good luck!
