This is usually an indication that, for instance, a form was generated with the 16-bit version of the Forms Designer (or 'Builder', depending on the version of Forms) and you're trying to execute it with a 32-bit runtime. If the client is 16-bit, it has to be generated by the correct version of the builder for that environment, and vice-versa.
We ran into this when someone accidentally installed a 16-bit version of the Forms Designer on a Win95 development machine. All the clients were Win95 and couldn't use the .fmx that was created. It's not SUPPOSED to happen with NT & 95 runtimes if both are using 32-bit versions of the tools, but I've heard that this isn't totally reliable (it gets flaky after a while, and the fix is to convert your .fmb to an .fmt and back again).
