Yes, there is potentially many more than one rows / patient_id and there could be multiple fields for some of the data items. The name of the procedures for the last year could be different from patient to patient and what we're trying to get is one record for one patient with all the information...
I tried a pivot table but was not able / did not know how to show actual data values for text fields vs summaries? Is it possible to just show the values in a pivot table vs. summaries?

Not sure what you mean.
Does your data have more than one row per patient_id?
One way would be to group by the patient_id and put the information to be displayed in the group header or footer.
