how can I "pivot" the data from a crystal report
how can create one row per cspp row for each patient_id using crystal reports. Can it be done using a pivot table / feature or in excel?

Yes, there is potentially many more than one rows / patient_id and there could be multiple fields for some of the data items. I tried a pivot table but was not able / did not know how to show actual data values for text fields vs summaries? Is it possible to just show the values in a pivot table vs. summaries?

Not sure what you mean.
Does your data have more than one row per patient_id?
One way would be to group by the patient_id and put the information to be displayed in the group header or footer.
