I consult with a medium sized organisation.
We have been using salesforce for over two years and the basic customer data is now 2 years out of date !!
Question 1: Is it fairly straight forward to upload into Salesforce a CSV file or similar.?? The data to be uploaded would be very simple. Not much more than customer name and address etc. The process need not be 100% automatic.
Question 2: Our Salesforce is hosted by a third party. Is this the norm?? I feel it would be better in house as we would have better control. (Also there are concerns about the confidential nature of our data. I.e. our data is highly visible to the hosting company.)

You can use any of the many tools salesforce provides for loading data. They have built in wizards for the easiest jobs. Click on setup and the top of the screen, then (if you are an administrator), go under Administration>Data Management and you will see a list of data management options.
Secondly, salesforce.com IS a hosted application. The leading hosted business application on earth. Your data is encrypted, and salesforce can only assist with, access your system and view your data IF you personally provide access to it for support purposes. Also, keep in mind that you and your medium size company have nothing on most of the Fortune 1000 that all use salesforce to house their confidential data, and all the SOX compliance that goes with it. Personally, it's a kin to either putting your data in a bank(salesforce) or sticking it under your mattress(on site)....... and yes, I know, there are still people who feel their money is safer under their mattress. :)
