Relinking an entity from Lead to Account

Hi all,

I've got a customer who has a custom entity, lets call it Customer Info, that is related to Lead, which has to be filled in with customer information for each lead before they are allowed to be come an account.

However when the lead is converted to an Account I need that custom entity, Customer Info, record to be linked to the Account rather than the lead.  Is there anyway to do this via relationships, workflow, code?
