Oracle R12-AP Online accounting could not be created

While posting invoice in AP I get error as
Online accounting could not be created for this transaction. Please inform your system administrator that the Create Accounting Program has encountered an internal error,
 whilst I run account support program and then ERROE changes to

Online accounting could not be created for this transaction. Please submit the Create Accounting concurrent program with the Report parameter set to detail to determine the problem
I run the Create Accounting process through Request, some time it get completed with Normal status and some time with warning

Please advise why accounting program is not running.
