what Paul has written is correct. If I understand you correctly you're asking also how to prepare an OSS message. This is not a note, but a message in the SAP OSS (Online Support System).
To prepare such a message is quite similar to preparing a call to a support desk:
- make a note of the system message or the abap dump
- make a note of the exact actions which have lead to the problem
- which steps have you already taken to try to solve the problem (have you read OSS notes - as described by Paul and implemented given solutions)
- make a note of the SAP version and SP level of the system
- etc.

HI,
OSS notes are used by SAP to issue corrections to known bugs to the application/system.
If you experience problems with the way SAP is working you can search OSS notes to see if the problem you are having is known to SAP. If it is then you can implement the fix (usually through code corrections) that SAP have provided.
If the problem is not listed you are able to raise a call with SAP to inform them of the problem, they should then get back to you with suggestions and/or solutions.
Not sure I understand what you mean when you say 'how can it be prepared', but in order to use SAP you need to be a registered customer and have an OSS user set up for you by SAP.
