In HR I use a typical Warning report. However, if an infraction is deemed to be relatively minor I use a Notice of Concern. Sort of in-between a Verbal admonishment and a full-blown Warning. Has anyone done similarly and if so can you share the form's content. I have a draft of mine attached.
