Insert data into Form and Eliminate duplicate data

1 - I have created 1 table and 1 Form with all the information on the WorkmensComp.png file attached. The majority Fields contains Combo Boxes. When I click on the drop down arrow on a Combo Box and choose existing name for example and save, it adds the existing name in in the Combo Box twice. If I add the same name again in a different record it saves the name for the third time.
2 - How can I prevent a Combo Box to have duplicate records in it?
3 - I was told that I need to go through a process called normalization, but it is too difficult for me. If I can just fix #1 above, I will be just fine. Unless you have some good examples with screen shots of normalization for me.

Please guide me in the right direction.

In the Design view, right click on the Combo Box and click on the Properties option. In the Proprties dialog, under the Data tab, there is a field called Row Source that will show the SQL query used to populate the Combo-Box something like this - SELECT [TABLE].[FIELD] FROM [TABLE]. This field can be edited and you can make it to show - SELECT DISTINCT [TABLE].[FIELD] FROM [TABLE]. That will show only the DISTINCT values of the field in the Combo-Box.
