Some cards don't fully support this conversion. I'd get a card with S-Video out.

However when i connect everything, sometimes nothing comes up, other times it comes up in black and white, with the computer screen appearing condensed and three wide across the tv. What settings am i missing?

S-video only supports up to 400 lines resolution, so you should set your desktop to 640x480 and see if that improves the display. If you are only getting black and white, that means the chrominance set of pins is not sending a signal: http://pinouts.ru/Video/SV ideo_pinou t.shtml The cable may be faulty, a pin may not be making a connection or is bent, or the adapter may not be working properly.
