How can I get my sound and usb back?

I have lost my sound and usb.  I have done a lot of troubleshooting so I suspect it is a MOBO issue.  

Soundcard and USB are listed as functioning properly in the Device Manager, but neither works through speakers or headphones.  None of my USB ports work.

If I go to Sounds and Audio Devices in the Control Panel, my device volume and speaker settings are greyed out.  I have found some examples of this on this site of the greyed out settings, but none that fixed the issue.

What I have tried with no positive results:
Total reformat and reinstall to factory settings.
Ugrade my driver version from the Toshiba support site.
Upgrade BIOS to current.
There is no option in BIOS to enable/disable on-board sound.
I cannot even successfully remove the sound device to reinstall it.  It shows despite uninstalling.
There are no hardware conflicts or errors.

Any Suggestions?
