Embedded vs. attached graphic in e-mail message

Can anyone explain why e-mail messages containing embedded graphics fails to display?  

Case in point: I've noticed messages originating from the 'Net with embedded graphics often fail to display once they arrive in Outlook.  We're running Exchange 2003 SP2, the Internet Message Connector and Outlook 2003.

I tested this 3 ways from my personal Comcast.net account:
 1)  With a .BMP file open, I copied & pasted the graphic into a new message that I sent to my
      corporate account; RESULT: message arrived successfully with blank box where graphic
      should have been

 2)  Next, I attached the .BMP file to the message leaving unchecked the Comcast option  
      to "display the attachment in the message body"; RESULT: message arrived successfully
     with the attachment (as expected)

 3)  Finally, I attached the .BMP file to the message and checked the Comcast option to "display
      the attachment in the message body"; RESULT: message arrived successfully with the  
      attachment embedded in the message.

How is my copy/paste in 1) different from checking the "display the attachment in the message body"  in 3)?  Is there a different encapsulation for one than the other?
