BES on SBS 2003 with 2 NICs - changes on BlackBerry handheld not synching to Exchange

I have a customer with a fairly simple Small Business Server 2003 setup:
Router - External NIC of SBS server | Internal NIC of SBS server - Internal network

I have installed BlackBerry Enterprise Server Professional software and set up a handheld device.  Any mail, calendar and contact appointments on Exchange are synchronised to the handheld without a hitch; but no changes on the handheld are synched back to the server.

I wondered if this is because the SBS server has two network cards with only one having the ability to allow MAPI connections.
