I'm don't really see much of SDM anymore, but you can do it with CCPE easily enough so I have no reason to suspect SDM is any different. Both start with a home.shtml file, so the configuration should be the same. Assuming you have your SDM files in the root of the slot1 flash, add the following line to your configuration.
ip http path slot1:
You'll need to add home.shtml to the end of your requests in order to engage the SDM, but it should be functional otherwise.
http://x.x.x.x/home.shtml
As for installing it, the easiest thing to do is install it to the internal flash and then move the files onto the slot1 flash.

copy flash:*.cfg slot1:
copy flash:*.shtml slot1:
