My internal phone line stopped working. I made sure the cable is good and the phone is working. So the problem was left on the phone jack. I want to make another jack and connect the wires but I dont know the color code for it. Also is there any special tool for that ?

The tool normally used with network/phone cable is reffered to as a crimper. The one below will work for both. http://www.cablestogo.com/
To rewire the jack, you should just need something to remove the protective coating for the cable.
