Two Questions about structured cabling

Dear Experts,
We want to implement  structured cabling on our network.
I have below questions :
1- does horizontal cable ( From Patch Panel To wall-plate outlet)
Has  the Same color pattern at two end sides? - if we use straight-through cables for  connecting our PCs to wall-plate and Patch Panel To Switches.

2- if we use straight-through cables for  connecting our PCs to wall-plate and
Patch Panel To Switches.
Do I connect our servers and routers to patch panel at first and connect patch panel to our switch or connect directly them to our switches? Does EIA/TIA 568B standard tell  about this?
