It's not overheating is it? Is the temperature where the 6500 resides at normal levels?

This is being tested in a Cisco 6506 Chassis with a 2500W power supply and a FanTray2.

Unfortunately I do not have any type of smartnet agreements and this card is at least 3 years old, I have no warranty left on it. At one point this card worked great, I think I would most likely have to buy a new SUP720 unless I can repair the one. Could there be anything hardware wise on the SUP that could be causing the card to crash? Like I said I can get it to boot, but only for like 30 seconds.

I'm not sure what you mean by the black connector bits. To be completely honest, I wouldn't try messing with the module as you risk permanently destroying the card and any chance of having it RMA'ed as well as risk damaging your 6500 chassis. A 9X5 Next Business Day Smartnet on the 6500 is going to be significantly less expensive than a new supervisor module.

Sounds like the module has failed. If you have a contract on the 6500, I would open a TAC case and I would expect an RMA to take place.
