Duplexing not working correctly?

I am trying to print a duplex job from Word to a Lexmark T630 printer with a duplexer using a Postscript driver.  I want to print this on pre-printed letterhead paper.  The printer instructions say that for a duplex job the paper should be face up in the tray. For a simplex job the paper should be put in face down in the tray.

So I try to print a 3 page duplex job.  Page 1 and Page 2 print ok. Page 3 ends up printing on the back side of the paper (The side of paper that does not have the pre-printed letterhead).  What am I doing wrong here?  All i want is to get Page 3 to print on the correct side of the page.
