scanning USB and external devices...

Hello! how does one get to really clean a usb memory stick... this is my scenario...

User A comes to me to clean the USB, i put it in my computer and scan it, Kaspersky removes viruses, user leaves happy.

same happens for User B,C,D,E,F,G ------ Z, but at some point my computer acts weird... and yes its infected... how come it got infected if I have my AV up to date and scanned all the usb I never opened the usb i scanned it 1st, and all the time.... so why do I get infected after a few scanned usbs?? this is not new, this has happened since forever... what would be the effective way to clean usbs without getting infected?
