I have never in my 10+ years in IT seen something like this.
We have a customer with two servers that crashed with the same error after rebooting (whether regularly or is safe mode):
Autochk.exe not found, skipping Autocheck.
Then it gets a BSOD with this message:
"STOP: c000021a {Fatal System Error}
The Session Manager Initialization system process terminated unexpectedly with a status of 0xc000003a
the strange thing is this happened on one server (call it server 1) about 1 week ago and it really seemed like a failed raid controller. We took an image of that server and restored it to a new server since they were due anyway and it worked great for a week with multiple reboots. Then it happened again on it's new hardware. I figured it was a issue with the image so I started a rebuild, after the rebuild I went to their other server (call it server2) and disjoined it from the old domain and rebooted, on reboot it had the exact same error. Server2 has never been imaged so that idea is out. We then had the genius idea of doing an acronis universal restore of an image of server1 back to it's new hardware and replacing the raid driver. It worked. I am super paranoid it is going to happen again though.
Other details, when I tried to get into the recovery console to do the various things I can do there windows was unable to access the C drive (with several different raid controller drivers). ERD Commander 2005 was unable to see the C drive, showing it as a unknown file system. It could however see the D drive which was the remaining space on the same array just fine. If I boot off of the Acronis TI 10 CD and poke around that tool can see all partitions just fine.
This was the case on both servers.
We tried MS support and the guy we talked to there could not get past being unable to view the C drive in the recovery console, then was telling us to uninstall things from the server. He could not understand that if I could actually boot the server to a point where I could do those things I would have fixed it already.
We talked to HP support and ran full hardware diagnostics on both servers from the smart start CD which came up clean.
We rebuild server2 since it was just a single application and a file server, server1 is running off of the image we restored with a different raid driver on it's new hardware
Goback is not installed nor has ever been installed on either server.
Server1's hardware was an HP DL350 G3 wtih a smart array 5i, it's new hardware is a DL360 G5 with a P400i raid card. Server 2 is a DL360 G4 with a smart array 6i. both servers are Windows 2003 standard with SP2 and current hotfixes. both servers have the Kaseya management software 4.8 on them. AV Software is symantec corp 10. Both servers were patched the preceding week.
Server1 roles: DC, file server, print server, exchange 2003 server
Server 2 roles: MIP and SQL server
Does anyone have any ideas as to what the heck happened here? I am out of ideas.
