difficulty getting firmware update applied to qlogic iscsi hba to become compatible with ESX 3.5

This all started a couple days ago when trying to install new servers to replace ESX boxes destroyed in indoor rain (note to others, do not adopt ceiling mounted ACs over your production rack)

Anyway, ESX 3.5 on two identical Dell 2950s. Identical Qlogic qle4060Es installed. Both hang on boot at loading vmkernel qla4022.o (options) after selecting boot ESX normally (not debug).

Some investigation and support calls to Vmware, Dell, and Qlogic said to update (if needed) bios and firmware to 1.13 and 3.0.1.33, respectively. Verified BIOS for HBA, I don't know the firmware rev.

I burned a bootable CD that allows me to change from A: to R: and then listed directory contents of R: (which is a Cd containing the DOS4GW, IFLASH, QL4022RM.bin and QLA4022.dl (fw update)). When I go to iflash(.exe) /ff (firmware instructions from qlogic's site), I get: Qlogic iscsi adapter not found. I know it's there because if I reboot, I can go into the BIOS of the card and view its information i.e. serial, bios version, etc.
