first we need to be sure that iptables are starting during boot:
iptables --list
check that you are ok with rules
iptables-save > /etc/firewall.conf
than save it into config file
echo "#!/bin/sh" > /etc/network/if-up.d/iptab
echo "iptables-restore < /etc/firewall.conf" >> /etc/network/if-up.d/iptab
chmod +x /etc/network/if-up.d/iptab
Then create a script so ifupdown loads these rules on boot.
after that reboot and check that iptables rules are loaded by running command
iptables --list
if everything is OK than add squid to during boot daemons for example by sysv-rc-conf (you should install this first) or manualy by linking from /etc/init.d/squid to runlevel of your choice.

make the iptables rule static (something like iptables store, or tell us what is your distribution), and make the squid start after iptables (need to know distribution).
