I have seen this occur if you are running a version of Open Manage that does not work well with the Perc / Sas adapter. you can either stop and start all 4 of the open manage services DSA... or try to simply reinstall open manage, take the custom install and make sure the storage component is selected.
If this does not work, upgrade to Version 6.1 of open manage

What if you put things back the way they were before? Since this is an ancient CERC controller, it wouldn't surprise me one bit that if you had a bad disk (like one that has a shorted power connector) then it would bring down the entire controller.
I do not believe that the CERC has tolerance like unique drive path interface chips, so the controller is more likely to blow on mechanical / power-related things. (Also check power in general, perhaps the newer drives are something that the older controller can not handle. )
Another idea, this controller was architected when the biggest disk you could buy was in the 100GB range. If you have more than 2.1TB worth of disk in the controller then the controller won't be able to handle it. (Dell MAY have a firmware enhancement for this, but since you didn't mention specifics on the controller model and firmware, then you'll just have to look it up on the dell support site yourself.
