Need remaining checkboxes to dim when specific one is selected

Hi,

I have a series of checkboxes that pull types of equipment from my database.

Example:
Lab Fume Hood
Particulate HEPA Filter
Portable Point Source Air Pickup
Welding Hood
N/A
Other

I need either ASP code or JavaScript code that will dim ALL remaining checkboxes when the N/A checkbox is checked. I would also like it to undim remaining checkboxes when N/A is unchecked.

I also need a text box to appear if OTHER is checked (and disappear when it is unchecked).
