php image verification form

I have used someone's php script to make the image etc. work. All I need to know is how to validate it, then continue with the form submission. The page I used was this http://www.php-mysql-tutorial.com/user-authentication/image-verification.php it doesn't appear to be working right now but...at any rate, he gives all the code, but also checks to be sure a user name and password are in a database. I am not doing that and can't figure out how to chop that part out of the code.....help please.
