Cannot get php form to insert data into mysql database.

I am a newbie at PHP and MySqL.  I am running IIS 5, PHP 5 and MySQL 5 on my XP laptop.  I have been testing a php form that would ask the user for input and then insert the data into an existing database table.  The database is called testdb and the table is called symbols.  I have attached the .php web page script also.  When I enter data and hit SUBMIT I get the following address and error:

http://localhost/%3C?=$_SERVER['PHP_SELF']?>

The page cannot be displayed
The page you are looking for cannot be displayed because the page address is incorrect.

Please try the following:

If you typed the page address in the Address bar, check that it is entered correctly.

Open the localhost home page and then look for links to the information you want.
HTTP 405 - Resource not allowed

IIS has the following settings:

.PHP --> C:\php\php5isapi.dll --> ALL

My MYSQL works along with PHPMYADMIN.  I successfuly used PHPMYADMIN to create the database and tables!
