I need to enter data into a web page

I have an excel file that needs to enter data into a web page.  Click a button, wait for it to load the results and copy the results into excel.

Below is the source code for the site, and my VBA so far.  I can get to the site, but thats about the extent of what I can do. I need to know how to enter the information into the blank feild.  And to click the "Query" button.

I have attached a picture of the actual web page if that helps to decipher the source code...
