alt-tab icon not showing correctly - other icons fine - XP and Vista

I have an application which has its icon set to an .ico file which contains the 7 necessary icons sizes for XP and Vista i.e. 256x256, 128x128, 96x96, 64x64, 48x48, 32x32, 16x16

Icons are displayed correctly throughout all XP and Vista screens but when using alt-tab to switch applications, the icon is not shown correctly.  It is pixelated and the colours are all wrong.

What do I need to do to ensure it is displayed correctly when using alt-tab?

Using VB and windows forms in Visual Studio 2008.

If all the colors are wrong maybe you are including more icons in there. Check the icon and make sure they're all 32bit?
