Accessing table from other database.

I am writing an application software that accesses two tables from different databases. How can I make an SQL statement that joins the two tables.

This is what happened:

Table1 is under Database1 (e.i. PC01sql)

code  char 1
desc  char 10

Table2 is under DATabase2 in another SQL Instance (e.i. PC01sql/app)

acct  char 2
code char 1
amnt numeric 9.2

Is there any way to link this table like using inner joins or cross join in SQL statement? Can we synchronize both tables? is this possible? Please help...
