With so many solutions, how can you tell which solutions are most likely to help you and which ones are not? SQL question for Friday

Hello experts. heres one for friday.

write a query to generate the following o/p for the given i/p.
can use any t-sql operators and functions apart for the "case" construct
for the best (subjectively) solution there will be a chocolate bar of your choice.

ok best means, short and neat .... can also be complex, as we are not seeking maintainability ...
for an extra bar, get the very first 4 lines o/p to say "0 - 3 months" - not hard but just to go that extra mile.

to save you time, get the i/p from this:
declare @t table (c int) declare @i int select @i = 0
while @i < 30 begin insert @t values (@i) select @i = @i + 1 end
select c from @t

i have one solution, but i am sure there are many others....
p.s. lots of "select"s and "if"s is neither neat nor short, so one select command should do
