Creating a SQL 2005 Database from a restore (backup file)

I am using SQL Server Management Studio Express 2005 and I am trying to create a database from a backup file.  I have done it before in here, but this time I am getting the attached error.  I have checked the file permissions, and it shows I have access, but I am obviously missing something.
