Deploying a form in Access 2007

I have a form built to run with my query, DB, and report. I've run Package and Deploy, but nothing comes up when I run the program. I have it set so the Runtime file goes with everything. It loads, but nothing shows.

When I open the original .ACCDB I get the following error:
You don't have a source code control program (such as Microsoft Visual SourceSafe) installed on your machine. The source code control commands are therefore not available
