How can i pick a month / year to store in a record

I am trying to fill a field with a mont/year i select. This is for a monthly timesheet. I have a form with a field for every day of the month and the hours worked that day. I want to fill the form for a chosen month.  I want something like a combo that states : "January 2008"   next line:"February 2008"  and so on. Later i want to select the list to print like all months of 2008 or 1 month of 2008.Start Free Trial
