Execute Combo Boxes from Command Button

Hopefully this is simple for an expert.  Have a form with a command button to display multiple performance records for an individual.  Need to be able to pop three combo boxes (or an alternative) to gather the information.  Combo box 1 performs a query and returns all names, box two is a calendar box to select the begin date and box three is a calendar box to select the end date for the display.  All dat then needs to be presented for printing.
