custom dialog box to confirm addition of new record from a form

hello experts..

i have an access form and i want to have a command button for it..

first off, i dont want to use the default command button provided by access.. i was wondering how can i write my own command button code? i tried an "insert into" statement,, but it just has this ugly look to it.. when the button is clicked it pops up a message saying "are you sure you want to append a row?",,, and to the average user thats just not too pleasing.. any ideas?? say i have..
field1, field2, field3.... all with some values in it.. any ideas on how to add them to a databse w/fields Field1, Field2, Field3??

also,
everytime the user does click on 'add new record', i want to pop up a confirmation box asking the user 'are you sure this is wat you want to enter, and on the dialog box show them all the fields that they have entered, and have a cancel and ok button to it..

any ideas experts..??
