Parameter Value dialog box when running report, where is it coming from?

OK- I've wasted about an hour on this already, so time to call in the big guns.   When I run my report I get the "enter parameter value" dialog box shown below.  I have no idea why.  Here is what I have tried:
1.) Running the underlying select statement DOES NOT give the dialog
2.) I have no hidden text boxes anywhere on the report.
3.) I have removed all report fields, one-at-a-time and run the report each time.  I still get the dialof. As soon as all report fields are removed, I don't get the dialog (just a blank report!).  As soon as I put a report field back - I get the dialog.
4.) I have tried a totally fresh, new report, I get the dialog.

WTF?

Maddening.

The dialog has no effect on the report.  I click OK, the report results are fine each and every time.
