The ISP has disabled all firewall options. The http server is up as it can be accessed on the inside net. The server fwall has been disabled.

Assuming the NAT is properly set up and that the server machine has no firewalling options set that would block port 80 requests and that the http server is indeed up and running, you may have to consider that your ISP doesn't allow subscribers to run servers and blocks that sort of access.
