Outlook Web Access is displaying red x's when loading the page

I have been using OWA on Exchange 2003 for 3 years without a hiccup ... until about two weeks ago when it started displaying only red x's (image placeholders) rather than the images.  I have checked that exchweb allows anonymous access, and have restarted the server to no avail.  Your assistance is appreciated.

Is everybody having that issue? Your probably checked this already but see if the images are actually still in the template folder. Did anybody try and install a different template for owa?
Has exchweb got any content in it?

What is the host OS? Windows 2003 or 2000?

Yes, everyone is experiencing the problem (from both inside and outside the firewall).  I checked the \exchweb\img folder and there are images in there, is that what you were referring to?  
We are using Windows 2000 and Windows XP.  The server is running Server 2003 and Exchange 2003.

This problem was resolved with the assistance of Microsoft.  The resolution was a corrupt password with a user account which is used to by OWA.
