How to allow website visitors request a catalog and submit their mailing address?

Hello Experts,
On our website we would like people to be able to request a catalog of our products and have it mailed to them. Is there a relatively easy way of obtaining their address information using FrontPage 2000 on a 2000 Server O/S? Would a database need to be involved or could we just have the info emailed to a specific mailbox? Not sure where to start.
